Job Summary:

The Director of Nursing Case Management will be responsible for overseeing and leading our case management department, ensuring the delivery of high-quality, patient-centered care. In this strategic leadership role, you will collaborate with healthcare professionals, physicians, and other stakeholders to optimize patient outcomes, enhance care coordination, and promote effective resource utilization.

Key Responsibilities:

- Utilization, transition, and care coordination background. Develop and implement case management policies, procedures, and protocols in alignment with industry best practices and regulatory standards.

- Lead and manage a team of case managers, providing guidance, training, and performance evaluations to ensure optimal departmental functioning.

- Foster a collaborative and patient-centered culture within the case management department, promoting interdisciplinary teamwork and communication.

- Develop and monitor departmental goals, objectives, and performance metrics to drive continuous improvement in patient care, resource utilization, and outcomes.

- Oversee the utilization review process to ensure appropriate and efficient utilization of healthcare resources.

- Collaborate with physicians, nurses, and other healthcare providers to ensure effective care coordination and timely discharge planning.

- Monitor and analyze data related to case management processes, identifying trends, and implementing interventions to enhance care delivery and efficiency.

- Stay abreast of industry trends, regulatory changes, and evidence-based practices in case management, integrating new knowledge into departmental operations.

- Serve as a resource and provide education on case management principles, policies, and procedures to staff members and other stakeholders.

Qualifications and Requirements:

- Bachelor's degree in Nursing (Master's degree preferred)

- Active Registered Nurse (RN) license in the state of Alabama

- Minimum of 5 years of progressive case management nursing experience in an acute care setting with 2 years in case management leadership, McKesson InterQual experience, and ACM preferred.

- Demonstrated leadership experience in case management or related healthcare roles

- Strong knowledge of case management principles, processes, and best practices

- Familiarity with relevant regulations and accreditation standards (e.g., CMS, Joint Commission)

- Excellent interpersonal, communication, and problem-solving skills

- Ability to work collaboratively with a diverse team and across different departments

- Proficiency in using healthcare information systems and technology
